Beginning the journey
Welcome. A little nervous is normal. We will walk you through paperwork, your evaluation, and what treatment looks like.
Once you have made your initial appointment there is paperwork to complete. If you would like extra time (maybe you are dealing with headaches and cannot do it all at once), stop by the office and pick up a packet — or finish it on your phone with our new patient intake.
If you prefer to do paperwork the day of the appointment, arrive at least 15 minutes early. Current patients can also complete functional outcome forms in English or Spanish.
