This website collects contact messages and, when you choose to submit them, patient intake and functional outcome forms. Outcome and intake answers include health information and are stored for clinic staff only in the password-protected CMSpro admin. They are not published on the public site.

We use the information to schedule care, score outcome measures for your therapist, and prepare a document your team can place into Prompt EMR. Do not submit someone else’s information unless you are authorized to do so.
